HOUSECALL PROGRAM

Our in-home care program provides medical care for patients following a hospital discharge or during a time which a patient cannot leave their home. This allows for continued safety, education, early identification of potential complications that may cause hospital admission and readmissions.

We work hard to collaborate with you and your primary care medical provider to offer quality health care for patients who cannot leave their homes to see their medical provider and we provide this in the comfort of your own home. We offer post-hospitalization visits to ensure the transition back home is smooth by providing healthcare to help you recover and manager your condition.

We provide general primary care for chronic and acute illnesses for patients who have difficulty traveling to their medical provider’s office. We can function as your primary care provider until you are able to leave your home. Our nurse practitioners provide holistic, comprehensive and exceptional health care in your home.
